ABSTRACT

BACKGROUND: The aim of this triple-blind placebo-controlled parallel-arm randomized clinical trial was to evaluate the clinical effects of Lactobacillus rhamnosus SP1 or azithromycin as an adjunct to scaling and root planing (SRP) in patients with stage III periodontitis. METHODS: Forty-seven systemically healthy participants with stage III periodontitis were recruited. Following SRP, the participants were randomly assigned to one of three treatment modalities; (1) placebo (n = 15), (2) probiotics (n = 16) and (3) antibiotics-azithromycin (n = 16). The participants were monitored at baseline, 3, 6, 9 and 12 months after therapy. Probing pocket depth (PPD), bleeding on probing (BOP), clinical attachment loss (CAL) and plaque accumulation (PI) were evaluated. RESULTS: All 47 participants completed the study. At 12 months, all groups showed significant improvements of PPD and PI (p < 0.012) irrespective of the treatment modality and without significant differences between the groups. Probiotics and azithromycin showed no added benefit in terms of CAL. While the placebo (p = 0.002) and the antibiotic-azithromycin (p = 0.002) group showed a significant reduction of BOP, only the placebo group revealed a significant reduction of CAL at 12 months follow-up (p = 0.003). The number of sites and teeth with PPD ≥ 5, ≥ 6 and ≥ 7 mm were significantly reduced in all groups at 12 months follow-up (p < 0.025) irrespective of the treatment regime and without significant differences between the groups. CONCLUSION: The use of probiotics or azithromycin as an adjunct to SRP failed to provide additional benefits in the treatment of stage III periodontitis. The benefits of these two treatment regimes as an adjunct to SRP remain unclear. TRIAL REGISTRATION: NCT02839408, 10/28/2017, Clinicaltrial.gov.

ABSTRACT

A high level of general and oral health are invaluable assets, a factor not always considered a basic human right for their better life quality. The mouth is a critical point of contact with the external environment, which is established when we talk, chew, swallow and when food digestion begins. From a perspective of the human condition, the mouth is crucial for the integration of sound, social appearance of the individual, and is one of the fundamental components of overall health. Therefore, not having an adequate level of oral health affects self-esteem, quality of life and people's general well-being.

ABSTRACT

ABSTRACT: The objective of this study was to determine the effect of the subgingival irrigation of chlorhexidine 0.12 % of the total anaerobic microbiota. Microbial sampling to 30 subjects with periodontitis stage II Grade B, in pockets with a periodontal probing depth > 4 mm. The subgingival irrigation was made with 5 mL of chlorhexidine in the test group and with 5 mL of distilled water in the control group. 24 hours after the procedure was obtained a second sample to compare. It was found that the subgingival irrigation with chlorhexidine at 0.12 % achieved a statistically significant decrease in anaerobic microbiota (p< 0.05).


RESUMEN: El objetivo del presente estudio fue determinar el efecto de la irrigación subgingival de la clorhexidina 0,12 % sobre la microbiota anaeróbica total. Se tomaron muestras microbiológicas a 30 sujetos con periodontitis estadio II grado B, en sacos periodontales con una profundidad de sondaje > 4 mm. Se realizó la irrigación subgingival con 5 mL. de clorhexidina en el grupo test y con 5 mL. de agua destilada en el grupo control. 24 horas después del procedimiento se obtuvo una segunda muestra a comparar. Se detectó que la irrigación subgingival con clorhexidina al 0,12 % logra disminuir en forma estadísticamente significativa la microbiota anaeróbica total (p< 0,05).

BACKGROUND: The oral flagellated protozoan Trichomonas tenax has been associated with patients with periodontal disease. However, no recent studies have been conducted on the prevalence of T. tenax in Chile. The aim of this study was to determine the presence of T. tenax in patients with periodontal disease, admitted to the Dental Clinic of the University of Antofagasta, Chile, through Polymerase Chain Reaction (PCR) amplification of the beta-tubulin gene. METHODS: An observational, cross-sectional study was conducted on 50 patients diagnosed with periodontal disease, 20 of them with gingivitis and 30 with periodontitis. T. tenax was identified by PCR amplification of the beta-tubulin gene. Associations between the protozoan and periodontal disease or the presence of risk factors to establish T. tenax infection were determined using the chi-square test and binary logistic regression analysis. RESULTS: T. tenax was present in 28 out of 50 (56%) of patients with periodontal disease, and was more prevalent when associated with periodontitis (21 out of 30; 70%) than dental plaque-induced gingivitis (7 out of 20; 35%). Non-statistically-significant associations were observed between the presence of T. tenax and age, gender, smoking habit or diabetes. Statistically significant associations were observed between the presence of T. tenax and periodontal disease, and between T. tenax and the Periodontal Screening and Recording (PSR) index. CONCLUSION: T. tenax showed a high presence in patients with progressive states of periodontal diseases. Consequently, T. tenax detection is strongly recommended in patients with periodontal disease diagnosis and with a PSR index greater than 3.

RESUMEN: Objetivo: Evaluar la eficacia de L. reuteri como adjunto en el tratamiento de la gingivitis. Material y Métodos: Se realizó un ensayo clínico aleatorizado placebo controlado en sujetos con gingivitis durante 3 meses. El grupo test recibió una tableta por día de la cepa probiótica Lactobacillus reuteri (dosis 2x10(8) UFC por día), el grupo control recibió las mismas tabletas pero sin bacterias vivas. La variable de resultado principal fue el índice gingival (IG), y las variables de resultado secundarias fueron el índice de placa (IP) y el índice de sangrado al sondaje (IS). Se realizó comparación intra e inter-grupos en el basal y al finalizar la intervención (3 meses). Resultados: Fueron incluidos en el análisis un total de 30 sujetos (15 test, 15 control). No hubo diferencias estadísticamente significativas entre los grupos en el basal (p> 0.05). Después de 3 meses de intervención se produjo en ambos grupos una disminución estadísticamente significativa en el índice gingival, índice de sangrado al sondaje e índice de placa (p< 0.05). Se detectó una significativa reducción en el número de sitios con IG 2 solo en el grupo test (p< 0.05). Conclusiones: El uso de tabletas de probiótico con L. reuteri como adjunto en el tratamiento de la gingivitis, produce una significativa reducción en el número de sitios que presentan inflamación más severa.


ABSTRACT: Aim: To evaluate the efficacy of L. reuteri as adjunct in the treatment of the gingivitis. Materials and Methods: A placebo-controlled clinical trial was conducted in gingivitis subjects for 3 months. Test treatment consisted of the administration of one tablet per day containing the probiotic strain Lactobacillus reuteri (doses 2x10(8) UFC per day), the control group received the same tablets but without live bacteria. The main outcome variable was the change in gingival index (GI), and the secondary outcome variables were the plaque index (PII) and the bleeding on probing (BoP). Outcome variables were compared between and within groups at baseline and at the end of intervention (3 months). Results: A total of 30 subjects (15 test, 15 control) were included in the analysis. No statistically significant differences were found between the groups at baseline (p> 0.05). Both treatment groups experienced a statistically significant improvement in the GI, PII and BoP (p < 0.05). There was a significant reduction in the number of sites with GI 2 only in the test group (p< 0.05). Conclusions: The use of probiotic tablets containing L. reuteri produces a significant reduction in the number of sites with severe inflammation.

Forty-seven systemically healthy volunteers with chronic periodontitis were recruited and monitored clinically and microbiologically at baseline for 3, 6 and 9 months after therapy. Subgingival plaque samples were collected from four periodontal sites with clinical attachment level ≥1 mm, probing pocket depth ≥4 mm and bleeding on probing, one site in each quadrant. Samples were cultivated and processed using the PCR technique. Patients received nonsurgical therapy including scaling and root planing (SRP) and were randomly assigned to a probiotic (n=16), antibiotic (n = 16) or placebo (n = 15) group. L. rhamnosus SP1 was taken once a day for 3 months. Azithromycin 500mg was taken once a day for 5 days. All groups showed improvements in clinical and microbiological parameters at all time points evaluated. Probiotic and antibiotic groups showed greater reductions in cultivable microbiota compared with baseline. The placebo group showed greater reduction in number of subjects with P. gingivalis compared with baseline. However, there were no significant differences between groups. The adjunctive use of L. rhamnosus SP1 sachets and azithromycin during initial therapy resulted in similar clinical and microbiological improvements compared with the placebo group.

Se realizó una intervención terapéutica en 20 féminas de 40-85 años de edad con linfedema posmastectomía, atendidas en los servicios de rehabilitación de los policlínicos docentes Ramón López Peña y Armando Garc de Santiago de Cuba, desde agosto de 2012 hasta septiembre de 2013, con vistas a evaluar la eficacia del tratamiento rehabilitador, para lo cual se emplearon técnicas rehabilitadoras de drenaje linfático, kinesioterapia, terapia ocupacional y se hicieron evaluaciones evolutivas con escalas de dolor, fuerza muscular, grado articular y funcionalidad. Se obtuvo que 90 [por ciento de las pacientes entre 40-59 años, con menos de 6 meses de evolución clínica, tuvieron mejores resultados al finalizar estudio, por lo que el tratamiento resultó eficaz(AU)

ABSTRACT

Las enfermedades periodontales (gingivitis y periodontitis) son un problema de salud pública debido a su alta prevalencia, su impacto en la calidad de vida y los altos costos que implica su tratamiento. Sus principales factores e indicadores de riesgo son compartidos con las enfermedades crónicas no transmisibles (ECNT). Además, la presencia de enfermedad periodontal en un paciente con ECNT puede contribuir a su exacerbación y/o desarrollo, a través de diversos mecanismos patogénicos, y el tratamiento de la condición periodontal genera una reducción de la inflamación sistémica. Debido a lo anterior, las enfermedades periodontales deben considerarse como una ECNT, y se debe trabajar en la creación, desarrollo e implementación de medidas de promoción de la salud y de prevención de ellas y participar activamente de las propuestas ya emanadas desde aquellas ECNT que tienen como objetivo a los mismos indicadores/factores de riesgo de las enfermedades periodontales.


Periodontal diseases (gingivitis and periodontitis) are a public health problem. They are highly prevalent, they affect life quality and their treament is expensive. Their principal risk factors and indicators are shared with chronic non transsmisible diseases (NTCD). Also, the presence of periodontal disease could exacerbate or initiate the development of a NTCD. Furthemore, Periodontal treatment results in systemic inflammation reduction. According to above explained, periodontal diseases should be considered as NTCD. It is highly advisible to focus on development, building and its implementation of periodontal prevention practices and communications. Moreover, it is advisible to participate in NTCD prevention programs, which targets same periodontal diseases risk factors and indicators.
